Requirements:
Board Certified (current)
Be involved in active patient care (min of 180 hrs/year)
Medical license in good standing
No prior legal history against license (i.e. settlements, probation, etc)
Experience with Impairment Ratings, using the 6th Edition of the AMA Guides (Optional)
Ability to provide prompt turnaround time on reports
